PLEASE COMPLETE THE MAINTENANCE PROTECTION PLAN AGREEMENT

I/we understand that by filling out this form we agree to the terms and conditions of the Gas Plus Maintenance Protection Plan Agreement. I/we may revoke this agreement, subject to providing notice to Gas Plus Inc. 30 days prior to the anniversary date of the plan, otherwise the plan will automatically renew for 1 year.

NOT COVERED: Cabinets, heat exchangers, compressors, nuisance calls, acts of God ( flood, fire, etc.), equipment deemed not in adequate condition, discontinued equipment or parts.

Gas Plus reserves the right to terminate any plan agreement at anytime. Customer subject to service call fee if the equipment is not eligible for plan.

Coverage for Furnace, Air Conditioner, and Boiler.

Coverage for furnaces, AC/heat pumps, and fireplaces.

Coverage for boilers, generators, and hot water heaters.

Annual maintenance, inspections, and scheduling reminders.
